The following sections explain in detail what can happen when telmisartan/hydrochlorothiazide and the aforementioned drugs are taken together.
 
Aliskiren
Combining telmisartan/hydrochlorothiazide with aliskiren may increase the risk of dangerously low blood pressure (hypotension), fainting, kidney problems (including kidney failure), and dangerously high potassium levels in the blood (hyperkalemia). In general, such combinations are usually avoided. If it is necessary to take such medications together, very close monitoring will be required.
 
Aliskiren and telmisartan/hydrochlorothiazide should never be combined in people with diabetes, because they may have a higher risk for these complications. Also, this combination is not recommended for people with moderate-to-severe kidney problems.  
 
ACE Inhibitors
Combining telmisartan/hydrochlorothiazide with an ACE inhibitor may increase the risk of dangerously low blood pressure (hypotension), fainting, kidney problems (including kidney failure), and dangerously high potassium levels in the blood (hyperkalemia). In general, such combinations are usually avoided. If it is necessary to take such medications together, very close monitoring will be required.
 
Diuretics
When taking telmisartan/hydrochlorothiazide with a diuretic, your blood pressure may decrease too much. This is more likely to occur when telmisartan/hydrochlorothiazide is first started. In order to prevent this interaction from occurring, your healthcare provider may change the dose of either medicine, start you on a lower dose of telmisartan/hydrochlorothiazide, and/or monitor you more closely.
 
Potassium Supplements or Salt Substitutes With Potassium
If you are taking telmisartan/hydrochlorothiazide along with a potassium supplement or a salt substitute with potassium, the levels of potassium in your blood may become too high. This can cause serious problems. Ask your healthcare provider about using potassium and telmisartan/hydrochlorothiazide together.
